    Colleagues often ask me the same question when they first pick up a diffuser fiber: "Which tip do I order?" It sounds like a catalogue detail and it is not. The tip length decides how much mucosa or tissue the uniform field actually covers, and ordering the wrong one means either a cold margin or wasting energy on healthy tissue beside the lesion. My rule is simple — match the emitting length to the longest dimension of the target, with a small overlap.

    A 5 mm tip is for a focal, almost punctate lesion; a 10 mm tip covers most single premalignant patches; 15 mm is my workhorse for tubular structures such as a bronchus or bile duct stricture; 20–25 mm exists for broad fields like a whole scalp or a hemi-bladder wall. The fiber gauge tracks the same logic — thinner 400 µm keeps the assembly flexible in narrow tracts, while 600 µm carries more power with less bend loss for larger fields.

    I validated this against our own cases. In the oral leukoplakia cohort treated with a diffuser fiber, the patients who recurred tended to be the ones where I had under-sized the tip by a margin; once I moved to a length matching the patch plus two millimetres of overlap, the twelve-month recurrence fell. The same thinking carried into gynecologic work, where I now size the tip to the cervical canal rather than guessing.
